Caregivers Provide Quality Services in the Home Setting

In today's changing health-care climate, it's comforting to know that high quality home health care is readily available.

A Medicare certified, State licensed program, Home Health Care provides intermittent skilled nursing and home health aide services that allow patients throughout the Phoenix Metropolitan Area to remain safely and comfortably at home. Our caregivers are dedicated client advocates, working with clients, their families, and their physicians to provide the highest quality care at home following a patient's discharge from a hospital or skilled nursing facility.

It is our mission to assist our clients whenever they need us. The Foundation's Home Health Program provides Registered Nurses on-call 24 hours every day, for clients' questions and concerns.

In accordance with physician's orders, our staff offers a wide range of skilled nursing care for those with acute or chronic illness. That includes wound care, medication management, client teaching, catheter care, and diabetic management.

Home Health Aide Services are also available, provided by a trained, insured, State-certified Nurse's Aide. Services include assistance with bathing, dressing and grooming, help with ambulation and eating, and monitoring vital signs.
