Transition Smoothly Back to Your Home
When a patient transitions from a hospital back into their home, it’s often a stressful period as many patients are re-hospitalized within 30 days. We aim to make the transition smoother with our ACTIVATE program. Available to MercyCare Long-Term Care (MCP-LTC), Medicare, Veterans, uninsured and undocumented patients upon discharge from any Dignity Health hospital, this program offers access to a nurse 24/7, a home visit, medication reconciliation, follow-up phone calls, and education for you and your family members.

The Top Goals of ACTIVATE
Hospital discharge planners select enrollees with these 3 goals in mind:
- Improve discharge transition through integration of hospital physicians, pharmacists, nurses and health plan advocates
- Identify patient’s unique needs following hospital discharge
- Coordinate community services to foster prolonged stability following transition

ACTIVATE Kindness Closet
One of the goals of the ACTIVATE program is to ensure that patients are safe at home, but not everyone has easy access to medical equipment. That’s why we opened the ACTIVATE Kindness Closet, located on the campus of each Dignity Health Hospital.
