Patient Readmissions Are A Problem
We like to think that the hard part is over when we leave the hospital after an accident or procedure. But unfortunately, for many people, that’s not the case—data from 2018 indicated that 14% of adults (or 3.8 million individuals in total) were readmitted to the hospital within 30 days. In addition to being costly to both individuals and healthcare systems, these readmissions are often dangerous because they put the patient at renewed risk of hospital-acquired infections or illnesses.
Various factors can cause hospital readmissions, and there’s no one-size-fits-all remedy to the problem. However, research has suggested that better care, support, and patient education during transitions from hospitals to home (or community care settings) could be effective in preventing avoidable hospital readmission.
FSL's ACTIVATE Program Is Helping Patients Reduce Readmissions
To address this problem, FSL launched ACTIVATE. This program provides extra support for qualifying patients after discharge from the hospital. ACTIVATE offerings include 24/7 nurse access, a home visit, assistance with medications, phone check-ins, and education assistance for both patients and their family members.
FSL Needs Medical Equipment Donations
To help make successful recoveries possible for the people we serve, FSL is putting out a call to the community for durable medical equipment (DME). Many individuals we serve face mobility challenges during their recovery but may not have access to appropriate equipment. Going without the recommended medical equipment could result in a prolonged healing process or a fall, which can be dangerous for elderly individuals.

- Wheelchairs of all kinds
- Rollators (walker with seat and brakes)
