How Your Compassion Helps Patients Get Back on Their Feet

José did something special for his 50th birthday last month. He stood on his own for the first time in months.

The small businessman owns a landscaping business in the Phoenix area. In January, a cactus needle punctured José’s boot and lodged in his right foot.

He tried to care for the wound himself. But after a few days, José saw the foot was infected. Emergency room doctors discovered he had gangrene and were forced to amputate all of the toes on his foot.

José says his faith — and kind donations to FSL — kept him going. “My whole life changed in one day and I didn’t know what to do. But with God’s help, all things are possible.”

During his stay at St. Joseph’s hospital, José was introduced to FSL’s ACTIVATE team, a group of dedicated patient care advocates who ease the transition from hospital to home. Through a vast network of community resources and dedicated 1:1 care, FSL has reduced hospital readmission rates to under 5% for over 1,000 patients annually.

Your support for FSL allowed Mandy, José’s patient care advocate, to secure his insurance through AHCCCS, Arizona’s Medicare program. That paid for months of antibiotics, physical therapy, and rehabilitation.

Mandy brought a donated walker from FSL’s Kindness Closet to his home and helped transport him to follow-up appointments. She also connected José to community groups that assisted with rent, utilities, and food.

By June, Mandy could hear more hope in José’s voice when they spoke by phone. He could drive short distances and do some light administrative work for his business.

José finally had some income coming in. But it wasn’t going to be enough to sustain him and buy the custom-made orthopedic shoe he needed to stand. Once again, you showed your compassion and supplied the shoe in time for José’s birthday.

ACTIVATE aims to not only improve the discharge process but also to prevent hospital readmission by addressing the unique needs of each patient and fostering long-term stability through a broad network of essential community services.

Our overarching goal is to prevent hospital readmissions by placing the focus on the whole person. We are committed to providing the support needed for patients to follow their prescribed care plans and actively engage in preventive measures. Through the dedication of ACTIVATE’s patient advocates, hospital readmission is down to as low as 2%.
